Attention All 2nd Period Teachers:
You will be receiving Consent to Release Information Parent Forms in your RED FOLDERS on Monday, November 28th. 

1. Please distribute to the student whose name appears at the top of the page.
2. Students bring home for Parents to bubble and sign.
3. Student returns completed form to ROOM 358, MS. FORGIONE.
4. Remember STUDENTS return completed form to Room 358.

• No payment associated with Medicaid will be affected.
• State and City are assessing who should be paying for service mandates of IEP's.

Students bring the forms home. 
Parents need to bubble YES OR NO, SIGN and return by hand to school. 
Students return the form to ROOM 358, MS. FORGIONE ANY TIME BEFORE DECEMBER 9TH.

The information is being used by the State and City to determine who should be paying for services related to students' IEP. 
The information DOES NOT EFFECT PAYMENTS MADETHROUGH MEDICAID.
